An investigation by the U.S. Department of Labor's Occupational Safety and Health Administration(OSHA) revealed that ADM failed to perform critical inspections and tests on safety equipment. This oversight contributed to the explosion that injured McElrath and two other employees. The Decatur site has a troubling history, with previous dust explosions and fires.
